Getting started

When it comes to getting extra care of support, it can be difficult to know where to begin. We’d love to help you get started, with the services and support that’ll help you live the life you want.

If you are considering a Home Care Package, we can assist with any of the steps below. For other funding options please contact our helpful team.

Step 1
You will need to complete and Aged Care Assessment

This is done through My Aged Care www.myagedcare.gov.au or by calling 1800 200 422. They will ask you some questions about your physical and mental health and your daily routine.
Step 2
Finding out about the outcome of your assessment

The Assessor will contact you and let you know if you are eligible for care and the level of services you can be approved to receive. Depending on your individual situation, this may be a Home Care Package Level 1-4 which covers ongoing needs, Commonwealth Home Support Programme or Short-Term Restorative Care.

Once you have been approved, you will be placed on a national queue.
Step 3
Working out what it costs

Once you are approved for a Home Care Package, you will be required to complete a financial assessment. This is done with Services Australia and will determine if you need to pay an income tested care fee/contribute to your care.

Step 4
Select your aged care provider

When you are approved for home care services, you can then nominate your service provider to provide your services.

Step 5
Accept your Home Care Package

Once your Home Care Package has been approved and assigned to you, you will receive a letter from My Aged Care. You will then have 56 days to choose a home care provider.

First Choice Aged care will work together with you to choose the services that closely meet your individual needs. We will develop a care plan together with you and confirm the costs. These details will be set out in a Home Care Agreement.

Before signing an agreement, it’s a good idea to seek independent advice from a specialised aged care advisor.
Step 6
Start your care plan

Now it’s time to implement your care plan. Our team will work with you in rolling out the services and supports in your home. As your needs change, we’ll work with you to provide holistic, best-practice care.

Help around the home. Stay living at home for longer with some regular or occasional help to keep your home safe and well-maintained, to shop for groceries or prepare meals.

Getting out and about – Support for you to get out in your local community, to attend social events, start a hobby or attend an exercise class.

Looking and feeling your best – help with daily tasks like showering, grooming or taking regular medication.

Short term programs such as respite care or extra care to help you recover at home after a hospital stay.

Whether you’re getting back on your feet after a hospital stay or need help with cleaning or gardening, we offer a range of services to support you, your requirements and your interests. These cover all levels of care, from low to high.
